How to Become a Patient
Qualifications:
Bradley County Residents (Dental and Medical), Polk County Residents (MEDICAL ONLY)
Ages 19 or older
Uninsured or under-insured
Total HOUSEHOLD income at 150% or below the federal poverty level
Financial Screenings
Drop by for a financial screening at our clinics to see if you qualify.
Screening times are Tuesday and Thursday from 9-11 am and 1-3 pm. Please bring the following items with you:
Proof of HOUSEHOLD income for ALL people working and/or receiving income in the house (pay stubs, SSI letters, bank statements, child support letter)
If you are not working, please bring signed AND notarized letters of support from those who support you.
Photo ID
Current lease, bill, or piece of mail with your name and address (to prove you are a Bradley/Polk County resident)
Insurance cards (if you have any)
Medication list
$25 cash to cover the first appointment
